    Unreported Hospital Errors

    In a recent study the Department of Health and Human Service reports that what percentage of hospital errors go unreported?

    a.  33%.

    The majority of hospital errors go unreported according to the Office of the Inspector General (OIG).  Errors included death from septic shock and severe bleeding from anticoagulants. In addition, only one of 17 catheter related infections were reported. These numbers could be significant since previous research shows that 14% of hospitalized patients experience a preventable adverse event.

    The ominous results of this study highlight a culture of punishment where nurses fear reprisal for reporting errors. It is imperative that nurses develop a culture of safety in order to protect our patients. In a culture of safety, nurses feel empowered to report adverse events and are supported by physicians, and administration in finding solutions to prevent events in the future.
